In order to enroll your child at C. H. Gullatt Elementary School the following documents must be provided.  A registration and placement on an active class roll cannot occur until all of the requested documents have been provided.  The data clerk or desginee will assist you if you are lacking any of the items.  Please be sure to read the information provided to you in regards to proof of residency.  Thank you for your cooperation.

1. Certificate of Immunization on the current Georgia form. This can be obtained from a Fulton County health clinic or your pediatrician.

2. Certificate of Ear, Eye and Dental Exam on the current Georgia form. This can be obtained from a Fulton County health clinic or your pediatrician.

3. Birth Certificate. This must be the original or an official copy with number and seal.

4. Social Security card or a waiver form.

5. Last report card (not for Kindergarten student).

6. Notarized Affidavit along with two valid proofs of residency which would include: Mortgage or Lease, current utility bills such as: gas, light or water. (Satellite, cable, telephone, cell phone) are not valid proofs of residency.

Proof of Residence Information
It is a requirement of Fulton County School System that all families must provide proof of bona fide residency within the boundaries of Fulton County and within the established attendance zones for individual schools. Bona fide residence is defined as the primary residence where the family actually resides. It is very important that proof of residency is current and correct. The numbers of students attending an individual school affect the distribution of funds to schools and the proper and effective use of facilities. We thank you for your cooperation with this important matter.

The following guidelines will be used to determine your bona fide residence within the attendance zone for C. H. Gullatt Elementary School.

At the time of registration and before a registration is considered complete and ready for enrollment at the school, the registering party (parent or legal guardian only) must provide two of the following as documentation:

* Copy of home mortgage book

* Current utility bill (gas, electric, or water)

* Apartment lease showing name of lessee

* Current bank statement

* Copy of home contract

* Receipt to have utilities connected

* Current pay check stub

* Homeowner’s / Renter Insurance registration card

If you live in a home where the electricity is provided, you must supply a letter on company letterhead from the leasing office attesting to your residency.

Generic leases (leases with out preprinted logo of the leasing company) must be accompanied by a letter on company letterhead attesting to your residency.

If you are staying with someone or rent from a private individual, the aforementioned items will be required from that individual accompanied by a letter signed by the lessee attesting to your residency.  A School Social Worker may make a home visit to confirm residency.
