Registration Times: Monday-Friday, 8:30-10:30 a.m.
When you come to register, please bring the following documentation with you. In order to save some time, you may bring in your own clean crisp copies of the following documents; along with the originals to be verified.
- Vital Records
- Certified Birth Certificate original with official raised seal and it must state the parent's name on it. (Hospital certificates are not accepted.)
- Valid GA Immunization Certificate Form 3231 must be updated.
- Eye, Ear, Dental Certificate Form 3300 which must be marked as follows: Vision, Hearing both passed and Dental Normal. NOTE: (If you are new to the Country or State you may be given a 30 day waiver for both 3231 & 3300 Forms.)
- Social Security card
- A Notarized Affidavit of Residence
- Documents must denote the parent/legal guardian name and have a current date
- Must submit two proofs of residence.
- 2 proofs of residency include the following: Lease, mortgage, current utility bill (gas, electric, water bill) homeowner's insurance, current pay check stub, current bank statement, or receipt to have utilities connected with address listed.
- Absolutely NO: cell phone bills, cable bills, or telephone bills are accepted.
- If child previously enrolled at another school, please provide the withdrawal form and/or most recent report card/academic transcript. These are essential for proper placement of student.
- Parent/Guardian Photo ID or Driver's License is required at the time of registration.
- Withdrawal Process
- All withdrawals are handled on site with a 48 hr. turnaround period.
- Parents are asked to come to the school, sign withdrawal form supplying the student(s) names, grade, teacher and withdrawal date.
- Parents must bring Photo ID and are asked to pay all outstanding fines before the release of documents.
