All Medication, both prescription and over-the-counter, must be brought to the clinic with an authorization form. Emergency medications must be brought in with authorization forms AND a Health Care Plan.

…be sure to have the medication authorization forms completed & signed by your physician and also signed by you, the guardian. Medications CAN NOT be brought to the clinic without the proper forms. These forms are only good for ONE school year.
Medications, both over-the -counter and prescription, must be brought in the original container. Parents, not students, must deliver these medications to the clinic.
If your will have any of the following medications at school, he/she will need a Care Plan to accompany the SHS1/ SHS2 authorization forms.
- epinephrine
- inhaler
- seizure medication
- insulin
