Closed for lunch daily from 12 noon to 12:30 p.m.
Registration
Monday – Friday until June 13th from 8:30 a.m. - 11:00 a.m. and from 12:30 p.m. - 2:00 p.m. After June 13th, registration will be on Wednesdays ONLY from 8:30 a.m. - 11:00 a.m. and 12:30 p.m. - 2:00 p.m.
Parents may pick up a registration packet any day of the week.
Documents Required for Registration:
o Original Certified Birth Certificate (with official raised seal)
o GA Immunization Certificate Form 3231 (marked “Complete for School”)
o Eye, Ear, Dental Certificate Form 3300
o Social Security Card or Waiver
o Two Proofs of Residence (has to include parent/guardian name and current address):
(a) Current copy of mortgage statement
(b) Current bank statement
(c) Current gas, electric or water bill
(d) Current paycheck stub
(e) Receipt to have utilities connected (no older than 30 days)
(f) Lease Agreement showing the name of the lessee (no older than 30 days)
(g) Copy of HUD-Settlement Statement (no older than 30 days)
