Reminder to Parents:

The school should be informed of any student who has a health or medical condition, chronic illness, or requires assistance for any medical procedure, treatment and/or problem, or takes daily medication(s).  Medication Authorization Forms and Medical Care Plans need to be completed every school year.  Required forms can be accessed on the Fulton County Website, school website or picked up at school.

Illness Policy

It is important for the well being of our students that a child with ANY of the following symptoms be kept home:
-fever of 100.4 or higher within the last 24 hours
-vomiting and/or diarrhea within the last 24 hours
-any infection
-any green mucous from nose
-any signs of pink eye (any discharge, redness)
-or any other communicable disease that could be passed to others.

A child must be free of fever/vomiting/and diarrhea for 24 hours before returning to school.


Health Care Plan Reminders

It is important that you and your doctor complete a new Health Care Plan prior to each school year to help us meet your child’s medical needs at school.

This applies to:

  • Asthma
  • Diabetes
  • Food Allergies
  • Seizures
  • Sickle Cell Disease
  • Individual Health Plan (for other illnesses)
  • Health Care Plan Addendum
