Required Documents to Register a New Student:

Two Proofs of Residency that you live in our district. This requirement relates to all incoming 6th graders as well as new students.

(see below for examples of proof)

  • Birth Certificate
  • Social Security Number
  • Eye, Ear and Dental Screening
  • Immunization Certificate
  • Previous school records (if possible for proper class placement)
     
    New student registration is now done online through the Fulton County Board of Education website. (www.fultonschools.org) Community Tab, then click New to Fulton. Once you have completed the online portion, please bring the required documents to our front office.

Proofs of Residency Verification – you must provide 2

  • One utility bill (water, gas or electric bill)

PLUS one of the following:

  • Home mortgage bill
  • Valid driver's License with current address
  • Home sale contract
  • Bank statement
  • Paycheck stub
  • Apartment/House lease
  • Current HOA statement

Final report cards will be held until all media center and textbooks are returned to school. Final report cards will be sent home with students on the last day of school.

If the iPad is lost and/or the case, charger brick, or cord are missing the following fines will apply:

Apple iPad – lost 1st offense: $250

iPad damaged  1st offense - $100

Case – lost or damaged 1st offense: $40

Charger brick and/or charger cord – lost

1st offense: $20 (for each piece lost)

Students can submit payment for lost or stolen devices and/or accessories to Crabapple Middle School (by check, cash or online by credit card).

Failure to return a devise or accessory (case/charger brick or cord) will result in a fine being attached to the student's account.  Failure to pay the fine will result in a hold on records/report cards and student will not receive a device the following fall until all fines have been paid.

 

High School Course Credit Letter

Students who are taking high school credit courses will receive an opt-out letter on Tuesday, May 23. This letter must be returned to CMS by June 2 if you do not want to accept the credit for the course. Failure to notify the school of declining the credit by June 2 will result in the default selection of accepting the high school credit on your student's transcript.
